  4. Operating Procedures:
    • Document all operational procedures, including startup, normal operations, temporary operations, and emergency shutdowns.
    • Operators should be aware of process operating limits and potential consequences.
  5. Training:
    • Review procedures, safety factors, and health hazards specific to job tasks.
    • Sessions conducted by competent sources with a strong focus on documentation.
  6. Contractor Safety:
    • Evaluate a contractor’s safety performance before starting a project.
    • Contractors must be informed of potential risks and health hazards.
  7. Pre-Startup Safety Review (PSSR):
    • Review safety procedures whenever a worksite starts back up (even for minor changes).
    • Applies to new and modified facilities.
  8. Mechanical Integrity (MI):
    • Ensure equipment is designed, installed, and maintained to prevent failures.
    • Regular inspections and testing are essential.
  9. Hot Work Permit:
    • Obtain permits for hot work (e.g., welding, cutting) in hazardous areas.
    • Ensures safety during maintenance and repairs.
  10. Management of Change (MOC):
    • Evaluate and manage changes to processes, equipment, or personnel.
    • Prevent unintended consequences.
  11. Incident Investigation:
    • Investigate incidents promptly to identify root causes and prevent recurrence.
    • Learn from mistakes to enhance safety.
  12. Emergency Planning and Response (ERP):
    • Develop and practice emergency plans.
    • Ensure employees know their roles during emergencies.
  13. Compliance Audits:
    • Regularly assess PSM program compliance.
    • Identify areas for improvement.
  14. Trade Secrets:
    • Protect confidential information while complying with PSM requirements.
